BILLING INFORMATION

We are committed to providing you with quality and affordable health care. If you have questions regarding patient and insurance responsibility for services rendered, please review some of our payment policy terms below. Self-Pay Option

If you are without insurance coverage, or are not insured by a plan we do business with, you are responsible for paying for any health care services provided. Please contact our business office directly for the current self-pay cost.

Copays and Deductibles

All copay and deductibles are the insured's responsibility and is part of your contract with your insurance company. Knowing your deductible and co pay amount is your responsibility and is set when you sign your agreement with them.

Health Insurance

If you are insured by a plan we are contracted with, as a courtesy, we will submit a claim for your visits. If you do not have an up to date insurance card, or if your eligibility has been terminated, you are responsible for the full cost of the visits.

Non-Covered Services

Please be aware that some, or in some cases all, services you receive may be non-covered and not considered reasonable, or necessary, by Medicare or any other insurances. In this case, you are responsible and must pay for these services in full.
